CLINICAL APPRAISAL
INSTRUCTIONS
Clients who have previously received a QEST4/ Asyra evaluation are not required to complete a new
Clinical Appraisal.However, completing a Clinical Appraisal Follow-up is necessary for the purpose of
updating information related to any changes in condition, medications, or supplements since the last
evaluation.
